Blue Ribbon Data due May 1

May enrollment data is DUE soon – Thursday, May 1st! – MANDATORY (Denver and Gilpin counties only send data in October)

Remember – Don’t worry if all enrollments are not entered before the May deadline. This data collection is for internal purposes only, and is not sent to National 4-H Headquarters in May.

  1. You can send your data by regular mail (diskette, CD or Zip) or by email (WinZip). Instructions for sending data via diskette or zip can be found at: http://www.ext.colostate.edu/ye/help/backup_youth.pdf. If you are burning your data to a CD, be sure your CD is a “CD/RW” drive not “CD-ROM.” Also make sure that you have CD burning software installed. Instructions for sending your data via WinZip are located at: http://www.ext.colostate.edu/ye/help/winzip.pdf. BE SURE WinZip is installed before you try this! (If not installed go to winzip.com and download the free trial -- call me if you need assistance).
  2. Whether you email or use regular mail, EVERYONE will need to fill out the Enrollment Verification Document: http://www.ext.colostate.edu/ye/forms/enrollverification.doc. The following methods are used to determine the accurate numbers that are provided to accounting. For Total 4-H Club Members -- open Blue Ribbon -- Select ES237 | ES237 Verification -- Check Members and Cloverbuds (this includes New, Return, and Terminated Status.) Click Preview -- when the report has generated go to the last page and that is the total club number, which you then multiply by 5 and list on the first line of the enrollment verification document. Special Interest numbers are found by Selecting ES237 | ES237 Statistical Report. Click “Test-Group Data” under “Testing Report” and “Member/Leader Stats” under “Report” type, click “Preview.” When the report is generated, look at “Page 1,” “Total Column” (Line #4 – Youth Participating in 4-H Special Interest/Short-Term Programs). The total number is listed on the second line of the enrollment verification document. The only printed report I need is the “Enrollment Verification Document.” DO NOT print any ES237 reports, I only need those in October.
*** PLEASE DO NOT SEND CHECKS WITH THIS FORM ***
PLEASE WAIT UNTIL ACCOUNTING SENDS YOU AN INVOICE
  3. If you choose to send your data via email, please fax the enrollment verification form to 970.491.7932. If you are mailing your data please send to me at 4040 Campus Delivery; Fort Collins, CO 80523-4040.

The Youth Partnership for Health

It's that time again! The Youth Partnership for Health, the youth advisory council to the Colorado Department of Public Health and Environment, is looking for diverse 14-18 year-old young people from across Colorado who are enthused about shaping the State of Colorado’s response to important teen issues.

Applications are due on May 2!

The Youth Partnership for Health began in 2000 and is an advisory group of teens that gives input and feedback on policies and programs to the Colorado Department of Public Health and Environment (CDPHE) on issues affecting teens such as substance abuse, violence, injury, abstinence, teen pregnancy, and fitness.

What is required of each young person:

  • A willingness to discuss ideas and opinions with other Partnership members, health department staff, policy makers, and other teens and adults.
  • An interest in learning about teen health issues.
  • Active participation at monthly meetings held in Denver. Meetings last three hours. Most meetings will be on Saturdays and members are expected to attend all meetings.
  • Verification of reliable transportation to meetings.
  • Signature of your parent/guardian.
  • Letter of recommendation from an adult sponsor (other than a parent/guardian) such as someone from your school, church, synagogue, youth serving agency or shelter.

Our promise is to:

  • Encourage adult and youth partnerships.
  • Provide a $10/hour honorarium or verify for your school’s service learning credit; reimburse each young person (or his/her driver) $.46 per mile to attend trainings and meetings; and keep information confidential.

Examples of work YPH members do:

Give feedback to CDPHE programs and committees; visit CDPHE funded youth programs; review applications for state agency funding; talk with policy makers about teen issues; serve as a trainer or speaker on teen issues; and interview staff for youth related projects.

How members will be selected:

We are seeking young people who represent different backgrounds, experiences and areas of Colorado.
